Follow this tutorial to successfully activate the submission form.

You may see a pale yellow bar appear on your browser window. Proceed to click on it as instructed.

When you click on the bar, a drop down menu will present you with 3 choices. Choose 'Allow Blocked Content'.

You may also get the following security warning as shown below. You must click 'Yes' to activate the submission form. Fill out the form and click the 'SUBMIT' button.
